How do you if the software protecting. Icon or meter?? Do you have to install extention or keep software open.

The way to know if Bitdefender is protecting you and there are no issues is as you suggested, by the system tray icon or the security widget. These are the 2 main identifiers that all is well, or if something is in need of attention. No other extension is needed.
As you may already know, the security widget can be enabled by right-clicking the system tray icon, or by going onto the main BD Dashboard and on the bottom left side, Settings/Security widget.
